We assessed the accuracy of our new infrared thermal imaging system for measuring whole-body energy expenditure (WBEE) in obese patients (n=30). This system is based on the principle that the core body temperature (Tc) is proportional to the amount of body heat radiated by an obese patient, since his whole body is covered with a heavy clothing load and the basal metabolic rate is increased. The variation of Tc was measured with a non-contact infrared thermography system (FT-816, Omron) during a 60-min spontaneous rest period. Tc, WBEE, and the respiratory exchange ratio (R) were simultaneously measured with a ventilated canopy metabolic gas analyzer (COSMED K4B2). The coefficient of variation of Tc was 2.1% (mean +/- SD) in the 30 obese patients. WBEE was significantly correlated with R, but not with Tc, Tc/Tc (rs=0.874), or Tc/R (rs=0.916). During the 60-min rest period, the mean R was 0.737+/-0.048 (mean +/- SD), and the WBEE was significantly correlated with R (rs=0.865). However, the absolute value of WBEE measured with our infrared thermal imaging system was about 40% of the measured WBEE with the metabolic gas analyzer in the 30 patients. We conclude that the degree of underfeeding cannot be judged by the absolute value of WBEE measured with our infrared thermal imaging system in obese patients.1.
